Information about you that may identify you and that relates to your past, present or future physical or mental health or condition and related health care services is referred to as Protected Health Information (“PHI”).
The Health Insurance Portability and Accountability Act (“HIPAA”) is a law that contains regulations that protects you and requires by law health care workers to maintain the privacy of your PHI. We also have the legal and ethical responsibility to provide you with notice of what we do with your PHI. This will be reviewed during your first appointment with Dr. Zadroga. We reserve the right to change the terms of our Notice of Privacy Practices at any time. We will provide you with a copy of the revised Notice of Privacy Practices by posting an official copy on our website, sending a copy to you in the mail upon request, or providing one to you at your next appointment. In general, very basic and limited PHI is disclosed in some situations with your authorization and in other situations without your authorization. The more common situations are listed below:
WITH YOUR SIGNED AUTHORIZATION FOR:
1) Treatment
2) Payment
3) When required by law
4) Research and data
5) Family involvement in your care
WITHOUT YOUR AUTHORIZATION FOR:
1) Reporting child abuse, elder abuse, adults with developmental disabilities, animal abuse
2) Clients that have been deceased for more than 50 years
3) Medical Emergencies
4) Protection of suicidal or homicidal persons
